Our Pharmacy Services department provides safe and effective medication management services in an environment where the pharmacist is valued as an integral part of the patient care team. As a part of the Pharmacy Services team, you would also be involved in activities which are progressing towards the approaching implementation of CPOE, BMV and eMAR.

We currently have an opportunity available for a Pharmacist.

This position, which reports to the Manager, will provide a range of duties and services including, but not limited to the following:

  • Monitor and review patient medication therapy.
  •  Consult and collaborate with physicians, nurses and all members of the healthcare team to ensure appropriate and effective medication therapy for all hospital patients.
  • Provide drug therapy consultative services and respond to drug information requests from all members of the healthcare team.
  • Provide individual patient/family teaching regarding medications.
  • Act as a resource to hospital staff regarding medication use, medication management practices, legal and financial aspects of medication use.
  • Assume leadership in the delivery of antimicrobial stewardship programs within the hospital.
  • Research and contribute to Pharmacy and Therapeutics monthly submissions and recommendations.
  • Collaborate with the Hospital Educator and Diabetes Educator in planning and delivery of staff or patient education programs.
  • Participate in audits of medication management practices to ensure compliance with safe medication use policies throughout the hospital.
  • In the absence of the Manager, Pharmacy Services, supervise the daily operations of the Pharmacy Department.
  • Current registration and a member in good standing with the Ontario College of Pharmacists required
  • A minimum of one year experience in a hospital pharmacy practice
  • Hospital Pharmacy Residency Certificate
  • Knowledge and familiarity with computerized and automated hospital pharmacy systems
  • Demonstrated ability to exercise independent judgement and to assess situations efficiently and effectively

  • Excellent communication, organizational, time management and leadership skills
  • Ability to work well within an inter-professional team and to foster effective working relationships
