POSITION: Behavioural Supports Ontario (BSO) Clinical Transitions Facilitator (Registered Nurse, Social Worker or Occupational Therapist)
Huron Perth Seniors Mental Health & Addictions Response Team
Temporary Part-time

POSITION SUMMARY: The successful candidate will work with older adults and their families/caregivers living with or at risk of responsive behaviours associated with complex and challenging mental health and addictions issues, dementia or other associated neurological conditions. The BSO Clinical Transitions Facilitator will serve as the lead clinician for transitions focused on supporting high risk older adults requiring intensive transition support during long-term care home (LTCH) admission and across the system of care. The transitions clinician will be a member of the Seniors Mental Health and Addictions Response Team (also known as the BSO Mobile Team) and will work in collaboration with acute and community providers to ensure seamless care transitions between care sectors and providers. This program will service Huron and Perth Counties which includes supporting individuals, their families and/or caregivers at many different service access points and at times transporting patients and or accompanying them on transport.

REPORTS TO: Manager, Seniors Mental Health & Addictions Response Team

DATE REQUIRED: As soon as possible

QUALIFICATIONS:
- Member of a regulated discipline including Social Work (BSW), Nursing (RN), or Occupational Therapy (OT)
- Current registered member in good standing with the appropriate Professional College or Association
- Regulated Healthcare Professional with 5-8 years’ experience preferred
- Experience assessing, treating and developing care plans for complex patients
- Experience working in both acute care and long term care settings preferred
- Specialized skills in assessment of delirium, dementia and mental health with knowledge of best practices in these areas.
- P.I.E.C.E.S., GPA, U-First, Montessori training certificates preferred
- Current certification in Psychiatric/Mental Health Nursing or other relevant Geriatric Mental Health or Gerontology certification preferred
- Valid Ontario Driver’s license with access to reliable transportation and Auto Endorsement Insurance for transporting patients
SKILLS/ABILITIES:
- Excellent crisis intervention, assessment skills and the ability to develop effective treatment plans
- Demonstrated knowledge with psychopharmacology
- Demonstrated adult teaching skills to educate of care providers
- Knowledge of treatment modalities and group facilitation is an asset
- Computer literate
- Experience working with Psychiatrists, Family Physicians & other professionals with proven leadership skills that foster engagement and strength based outcomes
- Willingness to travel throughout Huron & Perth counties and the SWLHIN and the ability to work flexible hours to ensure that patient care and program needs are met
- Proven ability to provide coaching/mentoring to caregivers/providers and community members
- Excellent verbal, written and interpersonal communication skills in addition to team building skills that enable rapport building with stakeholders
