POSITION: Education/Implementation Specialist
                        Primary Site: Stratford
                        Temporary Full-time (up to one year)

POSITION SUMMARY:
Connecting the Dots for Family Caregivers is a regional project aimed to improve quality of care and experience for family caregivers, patients and healthcare providers in Huron Perth (HP). The project is one of four funded projects in Ontario, funded by The Change Foundation. Six healthcare providers in HP are partners in this project: HPHA, One Care, North Huron Family Health Team, Star Family Health Team, Alzheimer’s Society, and the SWLHIN Home & Community Care. Partner organizations and caregivers are working together to create tools and resources that will help raise awareness about the role of the caregiver, while helping them feel more supported, valued, and engaged in their important role.
The Education/Implementation Specialist will be responsible for planning, co-ordinating, and delivering education within project partner organizations, as well as supporting implementation of the educational materials, resources, and tools. They will facilitate organizational learning and co-ordinate education activities by utilizing strong leadership characteristics, effective application of adult learning principles, change management methodology and successful coaching and mentoring techniques. The candidate will work collaboratively with project leadership, project team, staff, caregivers, and partner organizations, as well as The Change Foundation and the other funded projects within the Changing CARE initiative. The Education/Implementation Specialist follows all safe practices and procedures to support a safe patient and working environment.

DATE REQUIRED:          As soon as possible

QUALIFICATIONS:
- BScN/BN or Degree in Health Sciences/undergraduate degree in a health related discipline
- Adult Education Certificate (attained or in progress)
- Leadership Certificate with an accredited University preferred

EXPERIENCE:
- Minimum 3 years adult education and training experience
- Background in healthcare or public education role
- Preferred experience with educational program development, design, instruction, and evaluation
- Experience with using a variety of creative learning techniques and presentation skills
- Demonstrated commitment to ongoing professional development
- Preferred background with co-design principles, project management, and quality improvement

ABILITIES:
- Creative, flexible, and work effectively with diverse group of individuals with sound understanding of the many different adult learning styles and educational backgrounds
- Computer literate with a demonstrated competence in technology
- Exceptional communication, interpersonal, problem solving, team building and conflict resolution skills
- Proficient written, verbal and non-verbal facilitation skills
- Self-directed, positive, and professional work ethic and demonstrated initiative
- Effective time management skills; manage competing demands and deadlines
- Act as a project ambassador and promote positive public relations with both internal and external stakeholders
- Strong knowledge of and effective application of adult learning principles and instructional strategies
- Commitment to Interprofessional Practice, Collaborative Care Model, and Person Centered Care
- Experience with program development, implementation, facilitation and outcome evaluation
- Highly developed change management skills and ability to function as a change agent
- Experience working with patients and caregivers
- Effective personal/interpersonal skills;
- Communicate effectively in oral and written formats
- Excellent listening skills
- Leadership, group facilitation and presentation skills involving small and large multidisciplinary groups
